  • Geriatric Assessment - $200 (60 min appt.)

There was once an older adult named Sarah. She was in her mid-70s and had been feeling more tired than usual lately. She also noticed that she was having trouble remembering things and had become more forgetful. Concerned about her health, she decided to schedule an appointment with her Nurse Practirioner.

At her appointment, Sarah's NP recommended a geriatric assessment, which would involve a comprehensive evaluation of her overall health and well-being. Sarah agreed and was referred to a team of healthcare professionals, including doctors, nurses, social workers, and other specialists.

During the assessment, Sarah underwent a thorough evaluation of her physical, functional, cognitive, and emotional health. The healthcare professionals involved in the assessment worked together to identify potential health issues and functional impairments that Sarah was experiencing. They also took into account her personal goals and preferences.

Based on the results of the assessment, the healthcare professionals developed an individualized plan of care for Sarah. This included medication management, physical therapy to improve her strength and balance, and cognitive training exercises to improve her memory and cognitive function. The plan also included referrals to other specialists, such as a nutritionist and a social worker, to address her specific health needs.

Sarah was relieved to have a clear understanding of her health issues and a personalized plan of care to address them. She felt like she was finally receiving the attention and care she needed to live a healthier and happier life. Over time, she noticed significant improvements in her physical and cognitive health. She was able to regain her strength and balance, and her memory and cognitive function also improved.

Sarah's experience highlights the importance of a geriatric assessment for older adults. It is a comprehensive evaluation that takes into account an individual's specific health needs and goals. It can help to identify potential health issues, develop personalized care plans, and improve health outcomes for older adults.
